Weekend On-Call Admission RN Position Position Summary: As a member of the dedicated admission team, the Admission Nurse has knowledge and competency related to admission compliance requirements and the hospice admission process. Works closely with the attending physician, hospice medical director, patient, and family on the initial assessment, care planning and care implementation for the patient.Schedule: Would be on-call weekends for Admissions beginning Fridays at 5:00 P.M. until Mondays at 8:30

Hospice Field Staff RN/Case Manager Position Position Summary: The Staff Registered Nurse is responsible for assessing the patient/family/caregiver needs, planning for the care, implementing the Plan of Care, and evaluating the effectiveness of the care provided.
